Our Healthy Living Program educates our clients to utilize their primary healthcare; ultimately, bridging the gap in the communication between residents, pharmacies and primary care givers. Individuals may find it difficult to maintain his or her contact with their doctors or even adhere to their medication without safe supportive housing. It is important that each resident is offered an initial assessment.
We will assess the following:
• Current knowledge of their health condition
• Existing healthcare and any potential healthcare needs
• Adherence to their HIV medications and/or additional medications
To then provide the highest continuity of health care, social services, and counseling we will implement the following:
• Basic HIV Education
• Comprehension of Medications
• HIV Medication and Mental Health Connection
Our Sober Living Program addresses the devastating impact of addiction and alcoholism on all facets of life. We offer 12-step meetings, sober living skills, relapse prevention, and recovery education. We also offer aftercare treatment program referrals, if necessary, to help keep clients involved in recovery.
Elements of Sober Living include:
• Substance-Use Education
• Peer-to-Peer Services
• Alcohol and drug-free social and recreational activities
• Recovery Coaching or Mentoring
Drug Addiction and alcoholism are chronic, progressive and ultimately fatal diseases; our Sober
Living Program aids the transition into mainstream society.
